Before Booking Please Have:
1. YOUR REQUEST FOR LIVE SCAN FORM

You are usually provided a Request for Live Scan Form by the requesting agency or your employer.

Valid Identification
2. BRING A VALID FORM OF IDENTIFICATION

You'll be asked to verify your identity by presenting a valid form of identification (i.e. Driver License, State ID Card, Passport). NOTE: If your ID is expired, you can NOT proceed to get a live scan because an expired ID is unacceptable.
